Don't have any details other than the article, but it seems very careless and senseless.
I have an order to setting up for overhead that I remember as COCOBSHT. That is:
Cones - Set up a safe work area around the lift.
Overhead - Survey the entire overhead area for obstacles, obstructions and energized components.
Chocks - Chock the wheels.
Outriggers - Set the outriggers.
BS - Release the Boom Strap
Harness - Attach your harness to the mount.
Tools - Try to take everything you will need so you only go up once (I usually work alone).
I have a note next to the pistol grip to remind me to review this list before I go up. If we all did this, it would have prevented a few of the accidents that I have read about here.
