PMH Cabinet Wrong Phase

This week a linecrew was sent to change out a cabinet to a tx in a underground subd. They went to the cabinet and checked the phase color and markings. Then they went to the PMH cabinet and found the fuse that was marked to go to that cabinet. They isolated that run and went to the cabinet to ground it. Blew the fuse on the road ,not in the PMH . They finished their work while a service truck went to refuse and made it back hot. The next day they sent a crew up to try to finger out what was wrong. A crew went the next day to finger out what was wrong. What was found was years before who ever put in the cable had picked up 1 phase of a 3 phase and put a cabinet there . Instead of being fused in the PMH it was connected to the blades on the main feed. It has been turned over to engineering to move it to a fuse in the PMH. Don't bet your life on what the cable marking say,Check it for youself.
