Everything Wrong

Lineman working on 14,400 volt, three phase line. the Lineman was not wearing rubber gloves or sleeves. It was found later that the reclosuer was not turned off. The lineman made contact while working out of a bucket truck with his left shoulder blade on the center Phase. The exit wound was on his left had at the X-arm ground. The flash totaly engulfed him, causing burns to 30% of his body. IT WAS FOUND THAT THE LINEMAN WAS WEARING THE WRONG SHIRT WHICH WAS A POLY BLEND. He was off work for 9 months while they tried to seperate him and his shirt.
