while replacing a 600 amp solid blade porcelain disconnect with a lineman of 20 years experience. On a 4160 kva circuit grd y system I was side tracked by a copper connector that would not open all the way. After fixing the connector I continued to make the connection to the bottom of the disconnect. thinking that i connected the top of the disconnect we proceeded to remove the jumper. thank god the guy with 20 years of experience went first because he recognized that something was wrong by the sound of the arc he drew while lifting the jumper and slapped the jumper back on . what had happened was I didn't connect the top of the disconnect. the top side tap was laying on the top of the new disconnect and look like it was in the disconnect.. the morale of this story is always double check your connections and keep focused on the job at hand.
