Our company had two linemen burnt while working on some isolated secondary mains being augmented.
They where untieing the old conductors off pin insulators and the spans had uneven tension on them.
As they untied the last insulator the conductor tension pulled a span quickly and it bounced upward hitting 11kv primary conductors that where hanging low mid span, both linemen took the fault current through their bodies the man on a concrete pole taking the worst of it.
They where both severly burnt..the secondary conductor had no household services connected so their bodies where the only path for the primary current.
Always check the clearance mid span between primary and secondary conductors before altering the tension of agumenting secondary.
