A short wile ago a crew foreman put a reclosure on one shot to string in new single phase line over existing single phase 2400.The new spool had about 20 wraps left on it when the staples let loose and the wire unspooled causing the grounded wire to fall into the energized line. New wire was draped across the fence and the new wire was laying on the ground. The recloser never tripped out and the phase continued to burn. Took a volt check at the contacts on the recloser switch, which showed that the contacts never opened even though the switch was in the OFF/non-reclosing position. As a result no persons were injured, but a dog leaning against the fence in it's kennel was killed.
