SPANNER ON BUSS

About 2 months ago a linemen was dispatched to a substation (indoor) at a factory in Melbourne Australia. The report was there was a shifting spanner left on a live bus bar at the substation from a work crew doing some work on the substation previously how it was left there is beyond me. The linemen then decided to remove it from the bus-bar but guess what happened he touched the shifter that was on the red phase onto the blue phase causing a 415volt flash he is ok but has burns to his face lucky he was wearing glasses. He thought it would be ok because he didn't want the customer to come off supply. All of us make decisions on our journeys with electricity. My attitude is if it isn't safe don't do it your life isn't worth a spanner or anything that matter I don't want to die at work .
