A crew was doing a fiber optic upgrade in fairfax, VA. I was not present at the time, but was sent with a foreman to resume the job when osha had cleared it. What had apparently happened, the crew had set up the fiber reel on a slack span with a 19900 volt primary at the top. All the lines had a pretty good sag to them.........the cable line a bit taught as it was in a tree. What looked like had happened to me, was when the lineman was pulling out the fiber he raised the bucket over the tree, while the driver was moving forward, and was going to lay the fiber down in the tree when he cleared it as a common practice. He apparently didnt look up , not realizing there was a 19,900 volt primary with a sag in it over his head. It went in his arm and out of his back and down thru a tire of a T40. One of the lineman there that day said all he heard was the sound of a cannon going off. And didnt realize what had happened until they saw him slumped over the bucket.
