Lineman Falls

Lineman falls to death while assending ladder still under investigation.

The cross arm he was preparing to work on was burnt off the top of the pole and was swinging freely. The crew released the ties on the conductors but one conductor became jammed. While he was moving into position, up the ladder, the arms/conductors gave way, possibly striking him and he fell nerly 6m.

Has anyone had any similar experiences or know of a work practice that may prevent this from happening again.

We have issued a notice requiring either a replacement arm to be fitted or the crossarm secured prior to any work being carried out.

Any thoughts please.
